Updated upload/edit upload form setup

This MR modifies the JavaScript controlling the setup of the "Upload" and "Edit" upload pages to fix an issue highlighted by Sandeep:

Also when you update the dataset, the boxes for primary/secondary antibody don’t pop up unless you change the modality from dSTORM to something else, then change it back. Is there a way of allowing information on the primary/secondary antibodies seen when going back to updating datasets.
